Abstract

Background: The transverse colon, connecting hepatic and splenic flexures, exhibits biological characteristics of both right- and left-sided colon cancers. However, its precise classification remains controversial. This study aimed to evaluate whether further subclassification of transverse colon cancer (TCC) impacts prognosis.
Methods: A retrospective study was conducted on 358 patients with stage II and III TCC who underwent R0 resection between 2012 and 2020. Clinical baseline characteristics, pathological features, and prognostic outcomes of patients with right transverse colon cancer (RTCC) and left transverse colon cancer (LTCC) were compared.
Results: Patients with RTCC were generally older (72.0 [66.0-76.0] vs. 71.0 [65.0-75.0], P = 0.008), had a higher proportion of females (54.50% vs. 44.22%, P = 0.056), and larger tumours (4.5 [4.0-5.5] vs. 4.5 [3.5-5.0] cm, P = 0.003). No significant differences were observed between the two groups regarding recurrence-free survival (RFS) or overall survival (OS). However, subgroup analyses revealed opposite survival trends for RTCC and LTCC patients at stages II and III.
Conclusion: TCC patients generally exhibit multiple unfavourable oncological features. Although overall RFS and OS did not significantly differ, stage II RTCC may possess lower biological aggressiveness, suggesting potential clinical benefits from a more precise subclassification of TCC.
